I withdrew from a PhD program in the first semester due to personal reasons. Do I still have to submit a transcript of the classes I withdrew from in my app? Also, how does this reflect on me that I left the program within the first semester?

Yes. You have to submit transcripts from all schools you have attended. I won't speculate on how it'll look.

Definitely must submit. How it will affect you can vary greatly. How long ago it was and reasons for leaving will definitely be key. It's definitely a thing of concern when you see someone that went through the process and was given the opportunity to enter a program to just leave within 1 semester.
