Do we need to have the transcript request form with the transcripts or can I just request the transcript from my school and it will still be verified?

According to the AADSAS your application can be delayed without the form.

The form is so that your transcripts can be matched to your application when they arrive. Without the forms, the people at ADEA will not be happy campers when they have to manually find your application out of thousands 🙁
