if you click on "transcripts" on the main page it will say where they came from and date received

is there any kind of form to fill out or you simply give your school the address for AACOMAS in order to send transcripts?

there is no form. you just need to give your school the address (it can be found in the instructions part of the AACOMAS) :luck:
