i'm a preliminary intern at a program in New York City and so far I absolutely hate it. I was wondering if anyone knew whether it was possible to switch to a different prelim program, and how I would find open spots. Thanks.

It is fairly uncommon to switch in the middle of the year.

However, it can be done and must be done delicately because obviously you would be leaving your program (in most instances unless you find someone to take your place) short an intern.

Open positions can be found by:

- word of mouth
- listings on websites in that specialty (ie, Prelim Surg positions are often listed at apds.org)
- flyers sent to your PD
- trade journal ads
- cold calling programs

You will generally need a letter from your current PD stating that you are in good standing there along with the other usual application stuff (ie, CV). Your other option is to quit your current position and reenter the match - this would obviously entail repeating several months of your intern year, but if you are that miserable, it might be worth it. And of course if you have an Advanced position for next year, you might lose it if you ask to delay another year. Check nrmp.org to make sure there aren't any match violations if you decide to do that.

Remember that sometimes the evil you know is better than that you don't, or the grass isn't always greener on the other side.

seriously weigh whether or not you can stomach the next 9 months or not. if you can, stick it out. if you decide you can't make it another 9 months, take heed of the advice kimberli cox gave above.
