Transferring Programs

How does one go about transferring programs? Do you talk to your own PD first or contact the program you are interested in. Is there any way for the program you are interested in to "create" an extra opening? How do you even find out what programs have openings? I am asking for personal reasons/circumstances.

Issues:
#1. Programs are only granted a certain number of spots each year. This "creation" would have to come out of the programs own funds. Not likely to happen, I'm afraid.

#2. A given program is set up to accommodate a number of clinics with respect to the number of residents it has enrolled. Lose one resident and it throws the entire yearly schedule way way off.

I understand even the remote possibility of transferring is slim, and I also recognize that it would put a strain on my home program. I wouldn't be thinking about this if I didn't have to for personal reasons (with nothing to do with the program itself at all). Hopefully if the transfer occurs prior to the start of the year, any adverse affect could possibly be mitigated.
