HELP. I am scared! I have to be a subI soon. How do I advance from 3rd year to being a subI? I feel that I know nothing about how to write orders or do much of anything really. I feel none of the resources I've used has prepared me for this incipient challenge.

Here is an example: Bridge to warfarin using enoxaparin because S and C have shorter half-lives - I get the idea. How I make this actually happen in real life, I have NO clue whatsoever.

Ah, if you know the secret, please share!
