TSH high or low in Hyperthyroidism ?

If a patient comes in with hyperthyroidism and u measure his TSH.

would it be up or down arrow in the answer choices. I always get confused by these. High TSH= hyperthyroid

but when u have hyperthyroid, negative feedback stops TSH release so TSH would be down right ?

Depends on if it is primary or secondary hyperthyroidism. Primary hyperthyroidism with low TSH is far more common.

Primary hyperthyroidism: The thyroid decides to go crazy and make a bunch of T3/4. This exerts negative feedback on the pituitary, lowering TSH. T4 up, TSH down.

Secondary hyperthyroidism: The pituitary decides to go crazy and make a bunch of TSH. This activates the thyroid, increasing T3/4. T4 up, TSH up.

In clinical practice and on Step 2, primary disorders of the thyroid are so much more common (as the above user said) that you think of a low TSH meaning hyperthyroidism and vice versa.
