Tufts supplemental app?

I got a an email from Tufts, saying they got my application and to send in the supplemental fee. Am I supposed to send out anything else besides the check? There's no application to send with it... how are they are going to match up my check with my app??

send it in a letter containing your contact info
i.e. Name, Address, SSN, AADSAS # etc.
