TWU Recommendation Forms

So since TWU doesn't interview, they require 3 eval forms be sent in. I sent in the two required ones from my PTs and the third from a professor. But I just found out that my third PT that I didn't ask for a form is an alum of the program. Should I send in a fourth eval form or would that look bad to the application to to above three?
