Ok, one of the admissions people e-mailed me a few days ago saying that apparently there is a beige form that comes with my mailed application, but they have realized that they have forgotten to put that inside the package when they sent it to me. They asked me to send that in ASAP when it arrives.

Does anyone know what that beige form is?
