does anyone know how strictly the admissions committee look at the dental related experiences and volunteer stuff? do they check? like they ask you number of hours, phone number, and address? becuase i know some of my volunteer places that i've done several years ago and they've relocated or doesn't exist anymore. does anyone know???
 
I think the adcom looks at the overall package of the applicants. Just put down all your shadowing/volunteer experience even if you don't have their contact information anymore. If you are asked about your experience during the interview, just tell them what happened. They are usually understanding about the situation. Good luck!

UCLA's secondary does ask you to write in the hours you worked, your advisor's name, address and phone number. If they note any serious discrepencies in your application, they will probably bring it up in your interview or ask you for updated contact info.
