UCLA supplemental app with transcript matching form?

I sent UCLA supplemental app two weeks ago..but I am just wondering if I should have enclosed transcript matching form(from AADSAS) with secondary essay....I just sent transcript WITHOUT transcript matching form. Should I re-send it with transcript matching form?
Getting nervous!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

i dont think its necessary. I sent mine in without any form attached to it. I dont think ucla specified it on their secondary about a form that should be enclosed with your transcript anyways.
