UCSF Immunization

    How do we prove that we have done the required immunization?
    I have searched the ucsf health service website, but they only had
    Immunization from for Fall 2006. For those who going to attend ucsf
    this fall, how did you guys prove this? Do we have to send in any proof
    documents that we got from doctors for each shots?

    Typically you prove it simply by providing your immunization card. If there is a question you can get tested for MMR immunization and surface Hep B. ant.

    I just had a shot of varicella, MMR, TB skin test today and I took blood test couple of months ago, which shows that I have positive hep. B antibody.
    Now what i'm wondering is that I can't find the specfic Immunization form for Fall 2007. There is one for 2006 but I don't know if I should fill that one out.

    Why don't you call YOUR school and ask them?
