Anything I don't feel ok filling can be filled by the H1B pharmacist at Walgreens down the street
You'll actually get responses from the declined scripts, why would anyone fill these insane scripts?

I've seen oxycontin 80mg tid + percocet 10/325, so that's pretty close to yours.Ive seen oxy 30 mg #240 plus an additional oxy 10 #120 dispensed for the same person

Have a patient who uses Qvar nasally for some reason. Allergist insists on using it with some kind of adapter, rather than the myriad commercially available nasal steroids.

Probably the most ridiculous script I ever filled was for Methadone #1020. I don't recall the exact directions but it was a 30 day supply. I was floating, the patient was a regular, the PIC has a file on the guy that included a letter from the MD concerning the high dose. I can't recall anything else that crazy that I personally filled.

One day at work I went to fill a standard norco 10 script and noticed my stock was surprisingly low. Check my scripts and saw the floater the previous day had filled #500 1-2 q 4-6 prn from an MD about 6 hours away. Was pretty ecstatic about that.
I had a psych MD erx me a script with 7-8 controls on it at max dose, including 3 different benzos. I don't pretend to be an expert in psych disorders, but yeaaaa.

I've seen a weird one one somebody filled for Ambien...30 mg QHS. Patient had been taking it for some time at this dose and their prescriber claimed they were an ultra rapid metabolizer (doubt they included for which CYP), necessitating the high dose...

LTC pharmacist here. I have seen some pretty crazy things going on in nursing homes.
Fentanyl 50mcg one patch daily
Citalopram 10mg Q6h
Fluoxetine 30mg BID
All for the same patient:
- Norco 10 give three tabs Q4h scheduled
- Hydromorphone 8mg give two tabs Q4-6h
- Oxycodone 30mg give two tabs Q4-6h
- Methadone 10mg give 5 tabs Qam and Qpm
- Fioricet give two tabs Q6h
Those are just in the last week!

I once chewed out a dentist for prescribing nasal steroids to treat sleep apnea.
He told me I needed to learn my law because Texas allows DDS to treat sleep apnea. I agreed and hung up.
Turns out he was basing an entire clinic on a single line saying a dds can use a piece of dental hardware to treat sleep apnea, then explicitly stated that they needed to refer them to an M.D.
I just faxed him the law.
Apparently this DDS was advertising a sleep apnea treatment clinic and had been at it for a while with no-one challenging it.
