Hello!
I am a HS senior looking towards Med school. I have been accepted into Rhodes, Lewis and Clark College in Portland (close to home), Reed, Oberlin, Kenyon Occidental. I am looking for schools that train for med school application requirements.

Does ranking/reputation of under-grad school matter on a Med school application?

Among those? I wouldn't imagine a ton. Once you are at the top of the... basket... little separates them. Evaluate more important things like fit, closeness to home, and financial situations.

Also, are you aware of any Lewis and Clark under-grads getting into MD program? I have come across Rhodes, Oberlin under-grads in med-schools.

Does in-state versus out-of-state schools matter? I am from Oregon and Lewis & Clark is in-state. OHSU med-school is in-state. Rhodes, Oberlin, Kenyon, Oxy are out-of-state. I am dependent on parents' tax forms. If commit to out-of-state school and start filing my taxes separately do I become out-of-state for OHSU and in-state for say University of Tennessee school of medicine?

I cannot say that I have personally but a good education with a good MCAT score and a good resume I have no doubt you could get into an MD school with Lewis and Clark.
