Undergraduate Transcript req for any FM residencies?

I know some more programs in more competitive specialties require it (radiology comes to mind), but what about FM? I'm wondering because for personal reasons I'm strongly considering withdrawing from 3 courses this semester (in a non-degree year, have already been accepted to a D.O school and fulfilled its pre-reqs).
 
FWIW, I have to provide UG transcripts to get a license in certain midwest states. I can't remember if I had to do that for my first license...

Wow that's really odd! What purpose would your UG transcript have had...show that you have a undergraduate degree? It's not like they assessed it competitively or anything lol

Illinois does. I only know this because the ILDFPR couldn't complete my application in time because they misplaced the transcript, requring me to call and pay to have my transcript overnighted to me, then have me drive 3.5 hours each way to Springfield to hand-deliver said transcript, then have them go "oops, sorry, we had it the whole time."

At least I found out the Lincoln presidential museum is cool...

Yes, it is odd. I have no idea why they do it. My PD also says that the licensing boards are getting more and more specific regarding their questioning of them--when they used to just ask 'has this person ever been on probation or been terminated?', they now ask 'has this person ever had any remediation, probation, patient/parent complaint, discussions regarding professionalism, etc', which at some point could include any doctor ever.

I don't remember having to have the undergrad transcript for IL, but it was a while ago.

Requirements can be pretty silly though. I just filled out a license application that required both a notarized copy of my passport as well as a copy of my naturalization certificate. No idea why the latter, since the former has to mean I'm a citizen.
 
I know Ophto uses the sfmatch, and they need your undergrad transcripts. Not sure about any other specialty

Apparently my program sends it as part of their part of the packet. So it's not actually something that the applicant needs to provide themselves assuming you did your training here.
