I think it is unique because I cant find it. I have a course I took 8 years ago twice. I failed it both times and it is the exact same course. If I go back today and retake this course will It replace both fails from the past? Sorry if this is a heavily repeated topic.

Yes.
 
Yep, code the first 2 unsuccessful attempts as the "repeat" and the new attempt as "last repeat". For example, code the first 2 attempts as "R01" and your final attempt as "L01"
