FWIW I got a $650K job in Nowheresville in 2010 seven years out from residency.

Rad onc salaries are so stagnant that if we keep on this trajectory in about a hundred years you will make more money being, like, a fast food worker than a rad onc. (Exaggerating, but not much.) In the meantime I am really envious of these plastic surgery guys on Instagram advertising $250K per facelift.

The “9200 patients a year” comes more clearly into focus now

Rad oncs with professional service contracts have no bargaining position once admin realizes they can skim some of that professional by hiring someone … I mean the guy got “fired” for being two days late on an email reply. Turns into an Eric Bernian “Games People Play” transactional analysis (Now I’ve Got You You SOB variant) with the hospital as parent and rad onc as child. I’m reading between the lines that his new consult numbers fell over the years and his revenue generation ability fell since the IMRT heyday era. Rock the boat and be the squeaky wheel at your own peril!

The hospital recently lost a very similar lawsuit to the tune of $10M. I would be willing to bet in California's legal climate they will lose this one as well.

Still, at the end of the day, a costal California job so there is that.

Which it sounds like he squandered due to his “conscience.” For rad oncs circa 2026 trying to decide between a job and their conscience, screw your conscience to the sticking place as Shakespeare would say.

Seeing $2600 per day offers for busier locums jobs (consults, many pts under the beam)

I would just contact that center directly and work something out. I’m sure you could get 3-3500/day without a middleman. Seems like they have a need for a somewhat permanent locum vacation coverage position where they might even be willing to cover malpractice

We got a real problem if places like Turlock are considered "Nowheresville"

I see ads like this and think “I just can’t imagine they’re doing a bunch of HDR… and Cyberknife… and Gamma Knife… and IORT… in Turlock.”

This clinic will be paying the locums company more than it makes; on top of that its electricity bill, payroll, physics/dosi costs, etc. How does it stay afloat especially after the 2026 freestanding center cuts?

And... how in hell is it 8am-5pm with 5 patients a day?!

(this is a perfect example of where almost fully remote rad onc makes sense from a societal, ethical, financial, medical standpoint imho)

Presumably you're seeing 5 patients a day (like in say consults/follow-ups) and they're treating much more than that on a day to day basis

not bad if you can make it long term job/payment, esp if 'covering' with pager the other days also

View attachment 416412
$4,600 a day sounds solid.
3 day per week job. For a small volume center which this one is I bet that is ideal number of days per week to be on site. Can we acknowledge how unheard of (heretical?) this would have been 7 or more years ago.

AND AGAIN… the small volume center revenue goes down in 2026 and the locums pay significantly shoots up. Wild stuff.

It's really not that low volume, it can be a busy center. It's because the doc there has some health issues and is trying to retire, so it slowed down. This is probably a 20-25 patient on treatment with moderate hypo. I do locums for this system. They are desperate right now. If anyone wants to directly contract with them for a short term stint (a few months would help them a lot), let me know. It's going to be a great permanent job for someone that likes small town living and lake life. Not too far from skiing, either. Boyne getting about 4 feet this week! Even the other site that is busy is 3-4 days on site. The system is very forward thinking.

about 3K per day if you do this via a locum agency… sounds like they just trying to eliminate middleman
