I have one committee letter through AADSAS in which every recommendation letters are even from a dentist. But, there are 4 required letters of evaluation blanks in the UPenn supplement form. Is it okay to fill out one of them and leave 3 of them as blank?

I have one committee letter through AADSAS in which every recommendation letters are even from a dentist. But, there are 4 required letters of evaluation blanks in the UPenn supplement form. Is it okay to fill out one of them and leave 3 of them as blank?

The last 2 spots for evaluators both say optional next to them.
