Tracheal stenosis causes narrowing of the upper conducting zone, causing expiratory obstruction to air flow. This stenosis does not cause inspiratory obstruction as the expansion of the thorax causes sufficienct radial traction on the trachea to allow for non-obstructed flow. On expiration, the chest wall collapses and the stenosis trachea follows suit, causing obstruction.
TLDR; Intrathoracic obstructions cause obstruction to air flow on expiration and flattening of the expiration curve.
