Upward trend and health issues

During my first few years in undergrad, due to severe chronic health issues, I ended up accumulating several Ws as well as received several B's at this time with an average GPA of ~3.6. After several misdiagnoses, I was finally diagnosed with a chronic illness. Ever since the diagnosis, and finally receiving proper treatment, I have had a strong upward trend with ~3.9 semesters. However, I was wondering would my initial performance keep me from being able to attend a top school provided I did well on the MCAT?

Also, when applying would I need to explain the situation in detail, or would mentioning it in passing be okay? I don't want to make it sound like an excuse, but at the same time it did affect me during that time.

The situation would make for a good essay on a common Secondary essay topic: "Discuss a challenge you overcame," or "Describe a time when you faced a major stress and tell how you approached it."
