Urgent NYCOM secondary info!

I've been patiently waiting for my NYCOM interview invite for about 2 months now and I finally couldn't resist calling them to see what was up with my status. When I called them, they told me that they sent out a secondary two weeks ago to my home address. I've been home for an entire month and have received every secondary EXCEPT theirs, so somehow it got lost in the mailing process. Anyway, I'll be catching a flight out of the country tonight at 5AM from JFK, and am desperately trying to fill out the secondary without actually having it in front of me. I'm trying to answer every question in the form before I leave so that my parents (who are immigrants whose first language was not English) can simply copy and paste and send the application out for me while I'm gone. If anyone can add any specifics to the general questions that I have found from searching SDN, it would be GREATLY appreciated!

1. List all paid work experience:
2. List all volunteer work experiences:
3. List all research experience with dates:
4. List and describe your hobbies and recreational interests:
5. Indicate your extracurricular activities and the extent of your involvement:
6. List name and relationships of relatives in the Osteopathic profession:
7. How did you become interested in Osteopathic Medicine?
8. In the Space below discuss the reasons for applying to NYCOM

Btw, I got these from a post by Huntergatherer from 12/05.. hopefully everything is still the same.

My Questions:
1]--For question numbers 4-5, how much room is given for the description? How many hobbbies/extracurriculars should I mention for each?
2]--For question numbers 7-8, how long should the answer be? How much room is given? Attach extra sheets or no? Are they both in paragraph form?
 
well, for NYCOM, you only get a secondary if you are given an interview (i'm still waiting). but if i'm not mistaken, you bring the secondary with you to the interview. so all the freaking out you are going through might be unnecessary. the only thing is that i'd call to see when your interview is. hopefully it is not while you are away, and if it is, i'm sure they can understand the mailing mishap and accommodate you.

Yes this is true, they send the secondary with the interview invite and you fill it out and then bring it with you to your interview. They do not give you an interview date in the invite; instead you call and schedule. When I called the lady, who was very nice, gave me three dates. So just stay calm, try calling NYCOM to schedule an interview and enjoy your trip! The secondary form is very easy and since you will not be able to attend an interview until you return from your trip you do not need to worry about the secondary. Hope this was a little help!

I didn't realize that I'm supposed to bring the secondary with me to the interview. I thought that I was supposed to mail them the secondary, in order to secure an interview. So I guess I'll just call them after my parents receive the forms, schedule my interview while I'm on vacation, and worry about filling in the form after I get back. Thanks a lot! PHEW!!!! 👍

I was interviewed and accepted at NYCOM a couple months ago, so I hope all my information is still correct.
To answer your first question, they give you about 3 lines to write on. I ended up using one line for one activity (EMS, I'm involved in 2 agencies, with leadership positions, so I listed them all on one line) Then for the other I put the title of the EC and dates which I was involved. I placed commas between them. I didn't list all my activites, only the most important.

2-I can't remember exactly what I did, but I'm almost positive I printed out a seperate sheet and typed out about a paragraph for each.

How long did it take them to offer an acceptance ? I mean counting from interview date?

I Interviewed there around 11/7, the first week of interviews, and I found out within 2 weeks I believe.
