US-IMG applying to Canada

Several questions:

1) I am a US citizen, IMG and I am looking into going to Canada for Family Medicine residency.
2) I have already passed USMLE Step 2 CS, would I have to sit through the MCCQE Part 2?
3) Can a Canadian-certified physician apply to residency in the USA? How does that work out?

You need to be a Canadian citizen / PR to apply for residency in Canada.

I would second ptefabulous's question. It would be far easier for you to match FM in the US. If you still desired to practice in canada, you could easily do that later on, provided you attained board certification, and PR status in Canada.
 
It's better to be an FM in Canada than an FM in the US
 
is it absolutely necessary for an IMG to apply for residency in Canada before he can practice? or is it possible for an IMG to start practicing as a GP in rural areas once he has passed all necessary exams and not apply for residency training in Canada?

Depends on where you did your FM residency. Canada recognises FM training from a few countries, and the GPs from these countries do not need to jump through as many loops. Some of these countries include US, UK and Australia.
