At the end of the day there is no way to make yourself immune to someone suing you for very silly reasons. All you can do is take reasonable steps to make it very hard for the case to prevail.

If someone wants to sue you, they will. I don't live my life stressing about every single word of documentation because I know I practice well within the standard of care. If I did, my notes would be just a cut/paste of the UpToDate page for every medication I prescribe and a direct word for word transcription of my conversations with the collateral who tells me a previously suicidal person is safe to go home. Even then I don't think things like "withdrawal hypotension leading to falls in the elderly after stopping Prolixin" would be listed. So if I get sued for that and they win, so be it. I'll take my chances.
