Please note: I inherited this list from someone who applied in 2016, so best to check with individual programs to confirm whether policies have changed.

US schools with no prereq coursework expiration:

Davis
Colorado
Illinois
Mizzou
NCSU
Oregon
Penn
Purdue
Tufts
VAMD
Washington
Wisconsin

The other US schools have expirations or other restrictions ranging from 6-10 years from application or matriculation date. (Some schools will “review” courses taken more than a certain number of years ago, but not rule them out per se.)
 
I was applying with 11-14 year old coursework (undergrad 2001-2005) and I just wanted to second your recommendation to email the programs. I emailed all the programs I was considering even if their website stated there was no expiry date. I wish I could remember which ones, but a couple of schools sent me emails that read: "no, there is technically not an expiration, but we don't look kindly on it" and others seemed like they really didn't care about the age of grades and look at applications more holistically. It was a bummer to hear, but I was glad to have the information when deciding which schools to apply to.

I ultimately decided to only apply to my IS which has a 10 year cutoff for all science courses, but I was able to get a waiver even though the option of a waiver was not referenced on their website. So it's worth checking with programs that have a cut off listed, the worst they can say is "no".

I emailed UF about courses expiring and got this reply this morning. My lower level courses are from 1998-2004 but I am doing a Microbiology post bacc now to get the remainder of my prerequisites- so plenty of upper level sciences. This email definitely made my morning.

‘Yes, these courses will be accepted, as you will be taking upper-level courses now. It would be different if you took everything over 10 years ago.‘
