Using Real or Fake Names in Personal Statement

I volunteer for an individual who I mention in my personal statement. I feel like they will figure out based on his LOR and make the connection. I don't take care of this patient and if don't think he would have any problem with it.

Also should I makeup a fake doctors name? The doctor I am using is not writing me my LOR.
