USMLE Performance Profile Page??

Hey Everyone:

So SFmatch states they want "if available," the performance profile page listed on the back of the original score reports. I do not have my original report, and I assume that many people do not have it either. Is there a way to get it? If not, I guess we just should not worry about it? What are everyone's thoughts.

I saved my original detailed report as a pdf. I am submitting this.
