I am currently in my final year of MBBS. I am preparing for Step 1, but I have a question about the exam application process.
I have started the electronic application for the exam. I was told before by someone, a couple of years ago, that if I wanted to take the Steps before I graduate from medical school, that my school office needed to sign a hard copy certification that I've completed the basic science requirements, and that the school had to send the certification directly to ECFMG.
Is this still true?

From the ECFMG website:

Medical School Students
To be eligible for Step 1, Step 2 CK, and Step 2 CS, you must be officially enrolled in a medical school located outside the United States and Canada that is listed in IMED, both at the time that you apply for examination and at the time you take the exam. In addition, the "Graduation Years" in IMED for your medical school must be listed as "Current" at the time you apply and at the time you take the exam. An authorized official of your medical school must certify your current enrollment status; instructions will be provided at the time of application for examination. As soon as you graduate and receive your medical diploma, you must send two photocopies of your medical diploma and one full-face, passport-sized, color photograph to ECFMG (see Provision of Credentials and Translations). The photograph that you send must be current; it must have been taken within six months of the date that you send it. A photocopy of a photograph is not acceptable.

In addition to being currently enrolled as described above, to be eligible for Step 1, Step 2 CK, and Step 2 CS, you must have completed at least two years of medical school. This eligibility requirement means that you must have completed the basic medical science component of the medical school curriculum by the beginning of your eligibility period.

Although you may apply for and take the examinations after completing the basic medical science component of your medical school curriculum, it is recommended that you complete your core clinical clerkships, including actual patient contact, before taking Step 2 CK and Step 2 CS.

If you have passed the former ECFMG CSA, you are not eligible to take Step 2 CS, except under certain, well-defined circumstances. Eligible circumstances include: taking Step 2 CS to permanently validate an expired CSA examination date for the purpose of entering U.S. GME (see Validity of Examinations for Entry into Graduate Medical Education), taking Step 2 CS because your most recent performance on a clinical skills exam (CSA or Step 2 CS) is a failing performance (see Official Performance of Record for Examinees Retaking a Previously Passed Step in the USMLE Bulletin of Information), and retaking Step 2 CS to meet a time limit (see Reexamination and Reapplication and Retakes in the USMLE Bulletin of Information).

ECFMG reserves the right to reverify with the medical school the eligibility of medical school students who are registered for an exam. If your medical school informs ECFMG that your status has changed, and ECFMG determines you are no longer eligible for examination, your registration will be canceled. When a medical school does not participate in the ECFMG Medical School Web Portal (EMSWP), ECFMG's reverification of the status of the school's students may include ECFMG sending a written request for reverification by postal mail to the medical school. If such reverification is requested by ECFMG, ECFMG may cancel the student's registration or withhold the student's score report until ECFMG has received reverification of the student's status directly from the medical school. If your registration is canceled, you may be required to reapply.

So, they will confirm with your school, either electronically or on paper.

Good to know this works. I'm going to an MBBS program in China. doing 5-year Chinese MBBS instead of 6-year English medium. Because 5-year in native lang. is recognized by CA and 50 states, but 6-year one isn't.

Do ECFMG have staff that speak different languages? How will they read Chinese diplomas, transcripts and documents? How do they communicate with Mandarin-speaking school officials?

Okay I know I am replying to a thread that is pretty old... but my question pertains to this thread exactly and I don't think it would be appropriate to start a new thread before attempting a question here... so here goes:

I understood from the above post that while enrolled in a medical school program abroad one would need to get the application form for the USMLE signed and sent from the medical school itself.

My question is:

would I still have to get any document signed or anything done from my medical school, which is in India, if I wanted to apply for the USMLE after I have completed my coursework and received my final degree from the Medical school

or could I then just attach my degree to the application and that would be enough

I ask this because I am about to finish up my coursework in India and as I am a US citizen and resident, I don't want to have to come back to India every time i want to apply for a step...

You will need a Form 186 for the inital portion of your application. This form can be sent only by your college complete with the college seal, the Dean's signature and inside a college envelope. You can ask your school officials to help.

I see... So just to be clear I will have to get this form 186 for each individual step I am taking or is it a one time thing just before my first step application...

thanks btw for replying so quickly... didn't expect to get a response from such an old thread

you only need to get the paperwork completed once to sit Step 1, 2 ck, and CS. You will then need to get further documentation when you graduate (diploma and transcript and some other forms) to get your ECFMG certification. Then you can do Step 3 (no further paperwork needed).

Getting the paperwork done quite frankly is a lot harder and stressful than the exams! Well it depends on how good your medical school is but mine was fairly incompetent at the sort of thing. also the ECFMG is fairly incompetent and have a bad habit of losing things so you may end up having to send things more than once as did I!

When you apply for residency programs you will also require your medschool to send various things (transcript again and MSPE, and possibly but not necessarily LoRs) to the ECFMG.
