How well representative of the actual test is USMLE World? For example on the cardiovascular section, pharm is overly emphasized while on the repro section there are barely any questions on pharm. is that how the real exam puts weight on these topics?

Everyone's exam is different. You could get 40 cardiopulm questions on your exam but your classmate sitting in the cubical next to you could walk out having had next to nil. UWorld is fine with regard to its distribution.

The only thing I've heard with respect to UWorld is that it's where the money is at with respect to Q-banks.

And that it's a bit tougher than the actual exam.

In terms of my personal experience, from hardest to easiest:

UWSAs > real deal = UWorld > NBMEs
