i just did a question on UWorld with 68 y/o with brain metastasis with bleeding inside the metastatic area. He has melanoma, esophageal cancer, and likely prostate cancer...the explanation says prostate cancer doesn't metastasize to the brain? i thought prostate CA mets to the brain via batson's veins in the spinal cord...

thoughts??

(Edit, wow, I didn't read your post, yes the hemohagic melanoma thing is classic. But I posted all thsi stuff about prostate. I guess I see how important it is to read the whole question, or post...}

I just had a frustrating question too, maybe similar. Did the question give a list of most likely mets? Brain mets are not common at all with prostate ca. Spine met is mc.

see this http://www.asco.org/ASCOv2/Meetings/Abstracts?&vmview=abst_detail_view&confID=10&abstractID=242

also this, third bullet down, does explain they are rare. http://emedicine.medscape.com/article/454114-diagnosis

98% of Uworld questions are clear, some are not. (see my post above)

Just had this question as well, got it right only remembering that Melenoma commonly spreads all over the place, and vaguely recalling some path images of melenoma in vertebral bodies and and brain. As above, spine is most common site of prostate cancer spread, but it probably could spread to the brain via Batson's plexus. If I remember correctly lung is the most common cancer to spread to the brain.

also, melanoma mets to the brain are usually thought of as being hemorrhagic, though i think that is a bit tenuous a link.
