UWorld question on clavicular fractures

According to Pestana, Kaplan Surg notes page 15 and page 111, the treatment of clavicular fracture is figure of 8 device for 4 to 6 weeks

just had a UWOrld question about the clavicular fracture and asking for the next step in management and the answer was "angiogram"

explanation "a patient with a clavicular fracture should have a careful neurovascular examination to rule out injury to the underlying brahial plexus and subclavian artery."

So what is the next best step in management of a clavicular fracture, angiogram or go straight to teatment with a figure of 8 device???

You left out a very important part of the physical exam in that question and that there was a bruit heard over the fracture, thus indicating impaired flow of the artery and in that case yes an angiogram would be appropriate.

exactly!!! if the exam had no other abnormalities besides the fracture then you do a figure 8...
 
correct. unless of course the fracture was in the distal third of the clavicle in which case you take them to surgery b/c you're worried about nonunion issues...
