Value of an MBA?

How valuable is it to have an MD/MBA vs. just an MD? It seems like it would only increase your earnings potential if you went into businees. If you go into private practice, it might help you a bit in making everyday decisions about how to run your practice, but it won't necessarily increase your earnings potential, right?
What do people with MD/MBAs do? Are there people who get the MD/MBA and don't do a residency?
