I am having some troubles with the terminologies/ concepts and hope someone could clarify these

Fibrinoid necrosis vs. Necrotizing Granulomas
a) OK, necrosis is a death of a tissue
b) Fibrinoid and Granulomas are different right? I read before that Fibrinoid is specific to blood vessels? Granulomas is when it walled off the puss filled materials - BUT, what is the key difference here? What make them different?
(eg. Fibrinoid necrosis is seen in polyarteritis nodosa and Necrotizing granulomas is seen in Wegner - but why do they make sense?)
c) In other words, what should I think when I see granuloma vs. fibrinoid necrosis?
d) Focal necrotizing vasculitis seen in Wegner's disease a Fibrinoid necrosis????

Hypersensitivity in Vasculitis
a) I heard in Goljan that whenever you see Palpable Purpura, say Type 3 hypersensitivity. Is it always the case? Because in FA, it mentions palpable purpura in Churg-Strauss for which I thought is not type 3 because it's associated with Asthma and it has Granulomatous. I need clarification.
b) HSP is type 3 hypersensitivity - what other vasculitis are associated with hypersensitivity that are not mentioned in FA? Polyarthritis nodosa - type 3?
c) If there is a good association for hypersensitivity with different vasculitis, I'd like to know... I can't memorize them all 🙂

Thanks in advance!

As for vaculitis syndromes ,, all of them are necrotizing vasculitis .. meaning that there is vasculitis = inflammation of vessels
And necrosis.

Some types associated with granuloma formation like wegner .. and called granulomatous necrotizing vasculitis

Polyarteritis nodosa is characterized by fibrinoid necrosis .. which is only ch. Of malignant HTN and Polyarteritis N..
And give PAN characteristic nodular appearence and hence the name..

Some types of vasculitis are type 2 HSR like all types with ANCA antibodies
And some are type 3 like HSP and PAN..

All small vessel vasculitis produce palpable purpura whether type 2 or 3 HSR.. while thrombocytopenia produce nonpalpaple purpura..

I would really suggest pathoma lecture on vasculitides.. just amazing stuff
