Like the other person I am not sure if youre joking or not but none of those programs are community. I know about VCU for sure. Its a university program w/ the full range of fellowships and also has a complete range of residency programs. Its hospital itself is located in the city of Richmond making it very urban and academic.

JHU-Sinai is a separate entity from the JHU hospital program and I'm wondering how close is it's affiliation with JHU. Is it a community hospital that is owned by the JHU health system and do they share faculty?

I am not familliar with PSU but is a university based program. I do not understand the question about an urban pt population. If you want to be in a hip urban setting, PSU may not be the program for you. I would look at it this way. If you plan to do a fellowship you should go to the most competative program possible. You may want to be a PCP. That would change things , I suppose.
I worked at PSU Hershey for over a year between college and med school. Hershey is only 10 minutes from Harrisburg so and they see a good number of patients from there. Also, pts were LifeLioned (helicoptered) in from major accidents throughout the state several times a day. Don't know if this helps or not. Overall it is a less "urban" population, but because of its location patients often are sent there in emergencies (Philly too far, etc).
It is considered a top hospital, and I personally know several oncology patients who were told to go to Hershey over UPenn. Also excellent for cardiovascular work.
