I compiled a few years back of programs and volume. You have to also consider volume per resident. I've been told that volume is super important but then also been told to not look at volume at all, so it appears to be a contested topic and likely varies on the amount of that volume that actually gets down to the residents. These numbers came from my interviews and friends interviews and the info we got while at those programs. I don't know Penn or UCSF.
CCF - 1.9 million (8 residents/year)
UPMC - 1.4 million (11-13 residents/year)
Indiana - 1.0 million (15 residents/year)
Mayo - 1.0 million (13 residents/year)
MIR - 800,000 (17 residents/year)
UTSW - 800,000 (13 residents/year)
MGH - 675,000 (11 residents/year)
Wisconsin - 615,000 (8 residents/year)
Duke - 530,000 (12 residents/year)
Yale - 520,000 (13 residents/year)
Michigan - 500,000 (11 residents/year)
Hopkins - 400,000 (10 residents/year)
