I want the DO school perspective. I am currently holding an acceptance at a school A that starts in early August. I am on the wait list at my top choice, school B, which starts in late August. If I got accepted off the waitlist at school B after school A already started, could I still accept school B's offer? At what point is it too late for me accept school B? Further, how are loans affected in the process? I have to finalize financial loans in 1 week.
 
Once you start, you start. Regardless, it would also be way unfair to someone on the waitlist.
 
First year tuition is due at some schools within the first week. Most schools will take the tuition out of your dispersment (which gets sent to the school) automatically, so basically you could be out of one year's tuition if you choose to leave after the earlier school starts.

Honestly, I wouldn't think of doing something like that unless you have tons of money and that first choose is really far above your current choice due to real reasons like proximity to family/sick relatives or something that would really change your life (e.g. not for things like "reputation," which is not really very meaningful at DO programs).

I doubt he would be out a whole year's tuition. I had to pay 10k 4 times a year. I think, at worst, he'd be out 10-15k. But I don't know how loans work. I don't know if his loans would be stuck at the original school, so you might be right.

I would agree with your second statement. Some DO schools offer better training than others, but I think, in general, your opportunities are pretty similar graduating from any DO school.
