I realize this is a common trend right now, but would love to hear people's thoughts on these programs in terms of reputation, matching into fellowships, strengths and weaknesses. I am okay with living in any of these four places (not a big city guy).

Also, when thinking about this...at this point, I think I want to go into pediatric anesthesiology. So, garnering a peds fellowship spot from these programs is important to me.

Interviewed at all 4 of those places and would have been happy to match at any of them (and did). They're all strong academically and you shouldn't have a problem getting a fellowship out of any of them.

As far as geography goes, Charlottesville's probably the smallest and most college-y while Winston-Salem and Birmingham are more traditional Southern cities while the RDU area is kind of a mix of both.
 
i felt peds was weak at UAB, not sure how this impacts ones ability to get a fellowship there. prolly doesnt hurt much.
 
You would be doing great to get any of those 4 programs. All 4 have great reputations. Go where you want to live.
