MCAT Score: 503, (C/P:128), (CARS: 125), (B/B: 125), (P/S:125) [2nd Attempt]
504, (C/P:126), (CARS: 124), (B/B: 126), (P/S:128) [3rd Attempt]

State of residence: Florida, but I was born in New York.

Ethnicity and/or race: White/Italian

Undergraduate institution: University of South Florida

Clinical experience (volunteer and non-volunteer): Caretaker for my dad after his heart transplant and now Stage 5 Kidney Failure (2000 hours- still doing), Pharmacy Technician (500 hours), Cancer Patient Caretaker (1,000 hours), Cancer Patient Support Volunteer (750 hours), Lead Medical Assistant for Orthopedic Clinic (1060 hours), Medical Assistant for Weight Loss Clinic (240 hours- still doing).

Research experience and productivity: Sudden Cardiac Arrest Lab (1100 hours-still doing).

Shadowing experience and specialties represented: Plastic Surgery (300 hours), Pediatric Anesthesiology (25 hours), Immunology (25 hours), Invasive Cardiology (25 hours), General Surgery (25 hours), Neurology (25 hours).

Non-clinical volunteering: Cook and delivery driver for a food bank (800 hours).

Other extracurricular activities (including athletics, military service, gap year activities, leadership, teaching, etc): Tutor and teaching assistant (3660 hours- still doing), Marketing president and member of my fraternity (600 hours), Testicular cancer awareness advocate (1100 hours- still doing), Advocate for the American Heart Association (100 hours), Volunteer Lead for the 2019 Tampa Bay Heart Walk (10 hours), Co-Captain for USF Relay for Life Team (15 hours), Mentor for underprivileged kids in Tampa Area (50 hours), Mentor for autistic children (100 hours).

Relevant honors or awards: Made the Deans' List for six semesters at USF, recipient of the Bright Futures Scholarship, Winner of USF's Operation Global Action, and graduated Magna Cum Laude from the Judy Genshaft Honors College at USF.

Anything else not listed you think might be important: Stage 2 Cancer Survivor, battled obesity all my life and lost 80 pounds, watched my dad suffer from sudden cardiac arrest at 18, dad got a heart transplant, I speak three languages, grandfather committed suicide next door to me at 16, brother got a defibrillator due to a rare heart condition inherited from our dad at 18, mom lost her job from taking care of me while I battled cancer in NYC at 22, uncle died from a rare ear cancer two days after Christmas at 20, been in and out of hospitals all my life for testing positive for a genotype of a rare heart condition, and I scored in the 3rd Quartile on the CASPER Exam.

Toledo and Marshall admit few non residents with no connection to the state or region. You could add these schools:
TCU
Creighton
Rush
Rosalind Franklin
Medical College Wisconsin
Belmont
Eastern Virginia
Albany
Quinnipiac
 
You need more DO schools. You have taken the MCAT 3 time and I think this upcoming cycle will be your second cycle. Did you get any feedback from the schools you interviewed at? Are you aware many schools average multiple MCAT scores? (495+503+504=1502/3=500.666)
Your family has suffered profound tragedies. I hope you are successful at some point. Your MCAT stats are not competitive for MD schools so consider adding many more DO schools to your list.

USF doesn't even look at ORM's with <515/516 anymore. FAU screens around 507-508.
You can keep FSU, FIU, and Nova(MD and DO). UF is pretty holistic so I guess give it a shot if you did research there.
As others have said your MCAT is the only thing holding you back(would be great if you could get it to 511-512). Your story is powerful, so apply to the schools Faha recommended, but certainly add a lot more DO schools to the list.

Thats the thing, I have read many schools screen out applicants that take the MCAT more than 3 times, so wouldn't it be pointless to go for round 4 of taking the MCAT?

There may be a small number of schools who don’t like to see a lot of retakes(FAU being one of them) but if the score could go up a lot it’d be worth it. Another retake close to the same score would not look good, though.

I think the goal should be to make the best possible list this year and see what happens. You are competitive for all DO schools.

The only feedback, which you have already received from other users, is to add additional DO schools. You could add:
