You are not ready to apply with your ECs. Your complete lack of non clinical volunteering will limit your chances for interviews (volunteering at a bookstore is not what schools are looking for). Many schools screen at 150 hours of non clinical volunteering. You should accumulate 150+ hours in an activity such as food bank, homeless shelter, etc. before you submit your application. Your clinical volunteering is at the minimum for screening. Top tier schools are looking for applicants with hundreds of hours each of clinical and non clinical volunteering.

Separate your MPH GPA, especially from your science GPA.

What is your MPH capstone? It will not supersede the advice about having zero community service orientation hours, and you will likely be screened out of most schools. Having an MPH is not sufficient to show mission fit, and your purpose does not tell me why you can do this as a physician.

Thank you for your honest advice. To specify, the bookstore does serve underserved populations and has partnered with incarcerated folks which I was involved in. I also have ~80 hours of volunteer work with a different nonprofit that works to improve healthy food access in my area. Given these, would you still say I am not ready to apply / which schools should I be targeting from the list above?

Thank you for your comment! My undergrad cGPA is 3.89 and sGPA is 3.84. I had a 4.0 in my MPH program. My capstone was in pediatric dermatology.

You can apply, but concentrate on accumulating the 150 hours of non clinical volunteering I suggested in the coming year and add more clinical hospital volunteering in case you need to reapply. The majority of the schools on your list are not realistic with your ECs and MCAT scores of 514 and 516. At top tier schools you will be competing with applicants with many hundreds or thousands of hours each of clinical exposure and non clinical volunteering and MCAT scores of 520+. I suggest these schools:
Vermont
Quinnipiac
Albany
Hackensack
Penn State
Drexel
Temple
Jefferson
George Washington
Virginia Commonwealth
Wake Forest
Methodist (when it opens)
NOVA MD
Belmont
TCU
Alice Walton
Ponce (St. Louis)
Rosalind Franklin
Medical College Wisconsin
Western Michigan
Oakland Beaumont
Roseman
California University
Kaiser
Loma Linda (if you fit their mission)
