Are you implying that you have no interviews scheduled?

My concern is that you don't have any shadowing experience in primary care. Sure, you have some work in your free clinics, but I don't know if you have enough coverage of primary and emergency care in your profile. The limited exposure may not be enough to convince us you are ready for medical school. What is your purpose as a physician, and why is your only shadowing exposure in ortho informative compared to your free clinic/community work? Why not just lean on your success with establishing a mobile clinic and making sure it is sustainable? You won't have time to run it while in medical school.

With your updates, it does feel like you're checking a "mental health" box, especially 700 hours if you never mentioned it before in your initial submission. How does this add vs. your accomplishments with your mobile clinic?

Hello Mr. Smile. Thank you for your response. I haven’t received any interview invites. To clarify, the student run clinic is a primary care clinic that services primarily people who inject drugs and/or have mental health conditions. I am unsure as to what speciality I want to go into, however, I am sure that as a physician, I will conduct mobile clinic+harm reduction initiatives to serve people who are underserved, regardless of the speciality I choose. I shadowed in ortho as I wanted some experience in surgery following my EMT training and being very fascinated by the ER/OR. This ortho shadowing is the primary reason why I am torn between primary care/addiction medicine and a surgical specialty like orthopedics.
