You have a good application both activities and stats wise. Try to get a few more shadowing hours so you can really talk about the switch from PT to MD based on your observations of the medical field. Lack of research might hurt a bit for the tip top schools. You could list the research methods class as a research experience if you're presenting a poster/data outside of the class. Otherwise it's probably just a class no? You don't need to apply DO with your stats/activities. Your current list is a solid start. I would apply to at least 20 though.
