For DO, remove AZCOM (they target higher MCAT scorers). Replace CHSU with Campbell. They have a religious background and a good track record.

For MD, many of the schools you listed take few to no OOS students without significant ties. Working for someone who went to Louisville does not count. The only MD school worth applying to is UC Davis at your current stats. Maybe California University. Loma Linda might be worth looking into depending on whether you fit their stricter religious beliefs.
