Wait. Was there an IA or would you be self-incriminating? Making a mistake and learning from it, as you say you have done, is great but if there was not a formal IA or disciplinary action, you could potentially damage your chances.

My school technically took action, but it's nowhere in my transcript or record. My school doesn't provide committee letters. I'll talk to AAMC/more people about it, but for now I want to plan for the worst case scenario and assume it was a full IA.

Ok, that makes sense. You must report then since action was taken. It will hurt you, so kudos to you for getting ahead of it and preparing to address.

Any advice on the school list? With this special circumstance I really don't know where to start so I just went with my MCAT score/GPA.

Your app is extremely competitive save the IA. It is not an app killer but it will have some sort of impact. Verdict is out by how much of one. It would be an entirely different story if it were cheating, modifying records, etc. you get the drill. There are members from adcoms that volunteer their time and provide feedback on these forums, so perhaps they will provide their perspective on the situation.

State schools, many of the top 25, private. Without the IA, you would have your pickings on where to attend, certainly. Apply broad and use MSAR as a guiding source. I can’t recommend specific schools since I’m mostly familiar with just TX schools, but I am confident others are and they will likely respond with suggestions.

You're golden. The IA can be forgiven.
Good list! But unless you have been an OR resident, or went to school in OR, I suggest leaving off OHSU.

Suggest considering these: BU, Emory, Hofstra, Miami, Dartmouth. So few people get into Baylor from OOS that I suggest swapping it out for something else.
