You have a lot of teaching/tutoring/mentoring in your profile and very little service orientation activities. I can give a little credit as a coach for special needs kids, but I wonder if your choice has anything to do with your 6000+ hour hobby (like athletics/sports activities). As it stands, I see no service orientation activities explicitly listed (food distribution, shelter volunteer, job/tax preparation, transportation services, or housing rehabilitation). You need 150 hours in this category to avoid getting screened out at most schools, and given your metrics, you effectively need 250 to stay in the game for the brand schools on your list. Your application reads research-heavy so I wonder why not MD/PhD or PhD only since you have so many hours of academic competency activities (teaching/tutoring, coaching, and research) compared to your hospital/clinical experience.
