Comments at the end of the post. Thanks for the help! I apologize in advance if this is poorly worded or grammatically incorrect at times; I am in a bit of a panic.
cGPA and sGPA as calculated by AMCAS or AACOMAS:
Undergrad only (state school): 3.79 cGPA, 3.70 sGPA. I had a good upward trend and finished with three consecutive semesters of 4.0.
Toxicology masters: 4.0 from 21 credits so far, ~75% I believe count as BCPM.
MCAT score(s) and breakdown. Include all (non-voided) attempts:
520 (130/128/130/132)
State of residence or country of citizenship (if non-US)
Colorado for past 15 years, significant ties to Ohio
Ethnicity and/or race
White, non-URM
Undergraduate institution or category
Private colorado school, not-highly ranking
Clinical experience (volunteer and non-volunteer)
~2300 hours between 2 jobs over 2 years as an MA, all paid
Research experience and productivity
~600 hours over 1 year of wet lab, in vitro/in vivo public health research
Two pubs, not high IF, one 3rd author, one 1st author
A couple of posters/abstracts, one presentation
Shadowing experience and specialties represented
45 hours including critical care, pathology, sports medicine, and infectious disease
Non-clinical volunteering
~600 hours over 3 years with a community mental health initiative that provides free therapy to kids (I do not work as a therapist)
Other extracurricular activities (including athletics, military service, gap year activities, leadership, teaching, etc)
Restaurant manager, 1300 hours
Athlete (didnt compete in college)
~100 hours of mentoring an undergraduate student in my research lab as part of a training program
~800 hours working in a pharma internship in a clinical lab blood bank, did not list as clinical experience

Pell grant recipient
I'm very confident in my personal statement. I spent months and months working on it with significant input from current and past med students as well as other professional writers (my mom works in that field and has connections, I promise I didn't fork out thousands to buy my way into a good PS). I received feedback from the adcom for my current graduate program that my personal statement used for that application was the primary reason I got in, having no research experience prior. Obviously, the personal statement that I wrote for medical school is different, but the themes and experiences I touch on are the same. If anything, I think it will be better because I am not forcing past experiences to fit a grad school application, as they make far more sense as driving factors for medical school/wanting to be a physician. This is all subjective, of course, maybe it's hot garbage...

Any input would be greatly appreciated. I am worried that I am sort of delusional with my school list at the moment. I have a lot of highly ranking schools, but I don't know if I am a high-ranking applicant, per se. I know my MCAT is competitive, but my GPA is a little low. I am fairly confident that my activities are pretty strong, but maybe a little lacking in emphasis on medicine? The primary metrics that I used to build my school list were location and research funding/opportunities, as I am really interested in a dedicated 5-6 year program including research years, CCLCM as an example.
I am very particular about location, and I would prefer waiting a year over attending a school in an area that I don't want to be in (this is where I am really worried that I'm just going crazy and need a reality check). I have trouble in big cities, but I know that I will almost certainly have to make some compromises.
My biggest concern is that my list feels top-heavy. Any advice would mean a lot to me. Let me know if I missed anything or there is more information I can provide, thank you!
