MD & DO WAMC trying to figure out if i need a gap year/what to improve

Ideally would like to attend an MD school, but am open to apply DO as well.

GPA: 3.6 cGPA/3.4 sGPA
MCAT: TBD but aiming for 515+

EC's
2000 clinical hours as pathology lab assistant in a hospital (with patient interaction)
300 hours as an undergrad research assistant in an auditory lab
~50 hours for a semester as a gen chem II peer tutor/TA
150 hrs clinical volunteering in patient transport in a cancer center, pharmacy and ED in a community hospital
non-clinical volunteering at a farm (just started)
3 leadership positions in marketing/media:
- two are in a research student org/student research journal
- one was in a pre-health organization that did competitions at regional/state level as well as philanthropy
submission in a health humanities journal (IP)
hobbies include photography, writing, reading (goes along with leadership positions)
80 hours shadowing two specialties (IM and vascular surgery/hyperbaric medicine)

What are you doing with the farm? If this continuing in your gap year?

Your experiences are confusing when it comes to classifications. I really get a mixed sense of your expectations as a physician or your medical school experience. It looks like you keep yourself pretty busy but your why as a physician isn't clear.

i'm volunteering at the farm, it's a local farm. i basically help out the farmers with whatever tasks they need for that week, and i've been able to establish a sense of community there. i do plan on continuing this. could you elaborate a little more on why my experiences seem unclear?
 
It doesn't help us when you group activities together. I prefer seeing separated hours and specific key responsibilities because that's important for screeners. Specifically:

EC's
