You have a good DO list and should receive several interviews. For MD schools I suggest these:
VTECH
EVMS
VCU
UVA (unlikely but still in-state)
Georgetown
GW
West Virginia
Wake Forest
Drexel
Temple
Jefferson
Penn State
Hackensack
New York Medical College
Albany
Vermont
Quinnipiac
NOVA MD
Tulane
TCU-UNT
Creighton
St. Louis
Loyola
Rosalind Franklin
Rush
Medical College Wisconsin
Oakland Beaumont
Wayne State
Loma Linda
What is your strong connection to SUNY Upstate ?

The non-clinical community service needs some more work. I appreciate the communities when it comes to being a tutor, but those still fall under teaching. Need more info about Senior Center, including hours spent.

Thank you for the MD list help. Would you advise not retaking the MCAT? Or do you think it would be worth trying again? I was worried that since it was unbalanced it would not be competitive enough for MD, VCU/EVMS specifically. I am up for giving it another try with a better study plan but wasn't sure how big of a risk relative to the impact on my app it would have. I will be working Full-Time in the spring so would cut into my ability to write prewrite primary/secondary application(s).

For SUNY: Originally from the area. Several family members were treated/passed away there. Also, shadowed in part of their hospital system. I thought that justified a strong connection but let me know if I am mistaken.
If you have the time to study for a retake of the MCAT in the spring of 2023 it may be worth it. Do not retake unless your practice scores are consistently 512 or higher. The GPA-MCAT grid shows you have a ~80% chance for a MD acceptance and you are competitive for all DO schools.

The grid is not exact. If you had a 510, it jumps from 55% to 69%. Also the grid is 3.79 and above, and you are significantly higher. In the end, there is no way to know your real chances, as 16% of the top scores do not get in. However 24% of people with under a 2.39GPA, 506-509 MCAT were accepted, and a few people with less than a 486 were accepted.

Schools are considering factors outside of academic achievement.
