Remember when an entire ED chart with triage, RN notes, orders, H&P, MDM fit on one sheet?

I am trying to help med students learn H&Ps and would love to see your old paper chart templates: PDFs, docs, links, anything.

I think the thing you're looking for is called a "T sheet" or something like that, I just found a few templates on Google.

Vaguely recall someone talking about this in med school

seems they were just phasing them out

Oh man. Somewhere I have a whole bunch of t-sheets from my first gig out of residency. We had one for almost every chief complaint, and for some reason I saved them when we went to the EMR.
 
Oh man.
I remember T-sheets. We had a whole box of them. Grab the one for the chief complaint and circle the stuff. Actually check boxes WITH A PEN.
None of that clicky clicky computer crap. Unless it was your pen clicking.
I also feel very old right now.
 
Most ERs still prob have T sheets somewhere or on the computer. When EMRs go down, they have to chart somewhere. There usually is a back up system.
