Friday at about noontime.... let me count the weekend PMD 'dumps' so far !

Seriously? You don't find it ever annoying when the PMD reflexively tells their pt to show up to the ED w/o thinking about it? Or when the pt gets sent over for lab work or imaging because "it's faster"?
 
Yeah; the 'dumps' are frequently things that should be done on an outpatient basis, but the PMD puts the fear of god in the patient that they need to go RIGHT AWAY to the ED for their... (from yesterday, now)....

1. Recurrent outpatient paracentesis.
2. Repeat outpatient imaging study to assess for (whatever isn't really there).
3. Social admission for 'the dwindles' when they can really just be directly admitted.

I get paid the same whether I'm a chore-boy for PMDs or not, so yeah - they're pure 'dumps'.

Exactly ... your perspective on what is a dump and what is your kids college education changes based on your compensation model. I'm 100% fee for service in a market with decreasing volumes ... but I'm sure it would annoy the crap out of me if I was in your position.

Most of the stuff the PCPs send me are biscuits. But maybe I just practice in an area with good primary care.
 
We've got this weird nursing home dump situation where FM runs a certain NH and can warp core eject to the ED and if they require admission... it is barred from their service. They like...manage the place but all admits go to medicine. It makes for humorous admissions with much cursing and gnashing of teeth on the other line...on occasion. Luckily, it doesn't happen too often.
 
I only ever get "weekend dumps" in peds.....none of our adults are insured, so I don't know what a dump from a PMD would look like, lol.
