Western Supplemental Question

I have noticed that on a few supplemental applications (Western, A.T. Still, and a few others) that they ask you to list and describe your volunteer work. They ask for total hours worked, dates that you participated, etc.

Do these schools not even look at our AADSAS applications? If so, would it be ok to 'cut and paste' the same answers? They are asking for the exact same information, and I kind of feel like I have already said what I need to say about it. What should I do?
 
If they ask for the same info, put down the same info. If they are asking for more indepth answers then clarify what you did during those volunteer sessions.

I just copied those portions from my AADSAS. However I did elaborate on those experiences in the ASDOH supplemental because it was one of the essay questions.

For rewriting on the supp apps about vol exp, what should I do if I cannot remember the address, phone number of contact person or supervisor? I do not want anyone to question the work I did...Thanks!

if you don't want people to find out about the work you did, then you probably shouldn't put it down.

you can risk and put fake numbers, but you're on your own risk if they actually do call them and find out that you were not being honest on your application.

you can also try and leave it blank, but that'll make it look very unreliable and they probalby won't like it.

so in my opinion, don't put it if you don't want them asking about it
 
What sort of approach should I take with this section? I know there's a fine line between reasons and excuses, but am I suppose to tell a story here or list somethings?
